They can become enlarged from either constipation or diarrhea. When that happens, they bleed very easily. You will notice bright red blood in the toilet. There is usually no pain. Occasionally hemorrhoids will fill with clotted blood. This is called a thrombosed hemorrhoid. You will notice a hard lump on the outside of the anus. This lump will be painful.
